Our cognition of pleasant sensations is accompanied by the cognition of the unplaesant. When our mother gives us formula instaed of breast feeding us, we at once notice the difference and resist it. Our formative mind stores the unpleasant sensations felt by the body in its memory to be compared with other sensations to decide whether they too are pleasant or unpleasant.
